MISSION STATEMENT FOR HCBC WEB SITE

Two groups comprise this Home and Community Based Care Web Site:  Community Health Nurse Coordinators and Home Based Primary Care.  Both groups share the same goal:  to provide compassionate care to veterans at home and in the community, promoting maximal independence of the veteran and support to the caregiver.
