Community Care Coordination Service (CCCS)

What is CCCS?

First established in 1999, the VISN 8 CCCS provides governance and support to several major clinical programs across the network. These programs include Care Coordination Home Telehealth (CCHT), Care Coordination General Telehealth (CCGT), Store and Forward, Operation Enduring Freedom/Operation Iraqi Freedom (OEF-OIF), MOVE, Hospice and Palliative Care (HPC), and My HealtheVet (MHV).  The CCCS has established strong relationships with VISN 8 facilities that house these programs.  These relationships are collaborative and represent a partnership in advancing clinical care and services across all care settings for veterans.  These partnerships play an active role in the oversight of CCCS organizational performance to ensure compliance with strategic goals, standards and the dissemination and implementation of best practices across the network.

Care Coordination Home Telehealth  
Care Management System that combines care coordination and the use of technology to serve a variety of veteran populations.

Care Coordination General Telehealth
General Telehealth facilitates access and visits to their doctors for veterans in rural areas. By use of videoconferencing technologies and diagnostic equipment, specialists from VA Medical Centers can review patients in CBOC close to their home, avoiding travel and offering easier access to specialist care. Please contact your VAMC or your local CBOC for further assistance.

Operation Enduring Freedom / Operation Iraqi Freedom (OEF/OIF)
Continuum of care for VISN 8 OEF/OIF Veterans.  program components enclude access to care, care managment and care for Polytrauma & Severely Injuried.

Managing Overweight/Obesity for Veterans Everywhere (MOVE!)
The VA National Center for Health Promotion and Disease Prevention (NCP) has developed a VA weight management and physical activity program entitled MOVE! (Managing Overweight/Obesity for Veterans Everywhere)

Hospice & Palliative Care   (HPC)
Care in which the primary goal of treatment is comfort rather than cure in a person with advanced disease.

VISN 8 Telephone Triage  
The VISN 8 Telephone Triage Program provides after-hours, weekend, and holiday coverage throughout Florida, South Georgia, and Puerto Rico.

My HealtheVet (MHV)
MHV is a computer web based tool developed by VHA to support veterans and their families to make informed decisions about their health care.
