The
Extended Care and Rehabilitation Patient Service Line is composed
of four areas:
Physical
Medicine and Rehabilitation
(PM & R) serves to
improve the lives of
veterans through the
following services: Occupational,
Physical, Speech, and
Kinesio therapies. PM & R
delivers specialized
services to veterans
in need of rehabilitation
following amputation,
cerebral vascular accidents
(stroke), traumatic brain
injury and spinal cord
injury.

PM & R
has been selected
as one of four
national Polytrauma
Rehabilitation
Centers. Polytrauma
Centers deliver
specialized rehabilitation
care to soldiers
returning from
Operation Iraqi
Freedom and Operation
Enduring Freedom
(OIF/OEF) |

- |
A Spinal
Cord Injury (SCI)
Center is under construction. The program
will deliver comprehensive
specialized care
to SCI veterans.
The Center will be approximately 68,000 square feet and provide 30 beds and outpatient services. Construction will be completed in February 2009.

Extended
Care Center (ECC) is
a hospital-based transitional
care unit. ECC maintains
an average length of
stay of 32 days. Most
patients require extended
rehabilitation and care
following surgery and/or
lengthy hospitalizations
before returning to independent
living.
Home
and Community Care (HCC)
consists of programs
that manage care in veterans'
homes and community settings.

which incorporates
technology and
equipment to enhance
patient monitoring |

Other
programs are delivered
by community partners,
but the VA benefits
are managed by
the VA staff of
HCC. These programs
include Community
Nursing Home Care,
Community Adult
day Health Care,
Skilled and Unskilled
Home Care, Home
Hospice Care, and
Home IV therapy. |
The
Geriatric Research Education
and Clinical Center (GRECC)
integrates models of
clinical care, education
and research to further
the care of geriatric
patients with dementia.
GRECC offers education
to staff through the
Veterans Integrated Service
Network (VISN 23). The
GRECC's Memory Loss Clinic
is a key component in
the continuum of care
of patients with Alzheimer's
disease. Research activities
focus on the development,
diagnosis, and treatment
of Alzheimer's disease
and other dementias.
