Care Coordination Home Telehealth

A component of Care Coordination Home Telehealth (CCHT) Business Model is the generation of capital dollars for program expansion through the application of grant funding and we are effecting our plan as follows:

In FY 2002, the Business Office applied and received grant funding for two important demonstration projects:

• National Cancer Institute (NCI) – Advancing Cancer Care Through Technology Project (ACCT) (PDF)

The Veterans Health Administration (VHA), building on its experience with the use of technology in home care (Telehomecare) is in a unique position to test the use of Telehomecare with cancer patients. This proposal is a collaboration between the VHA, the National Cancer Institute (NCI) and the National Institute of Standards and Technology (NIST) to develop and test an evidence-based model for home-telehealth with cancer patients. This proposal is a one-year project.

In FY 2003 this proposal was a multi-VISN site collaborative project that includes VISNs 1, 2, 8, 11 and 17, to support the continued development, deployment, and evaluation of the innovative Care Coordination / Home Telehealth Model. This proposal will incorporate care management principles and the effective use of technology.
