Integrated Primary Care
The quality of patient care improves when patients are cared for by an interdisciplinary health care team of primary care and behavioral health care providers. Recent research has demonstrated that the physical and mental aspects of health are significantly interrelated. For example:
- Nearly 60% of physician visits are for problems that have a psychological component
- Sixteen of the twenty most frequent diagnoses by primary care physicians have lifestyle elements that behavioral health providers can address through prevention, or direct interventions
- Thirteen of the fifteen leading causes of death, including cancer, heart disease and diabetes have psychological components that, if addressed, will slow disease progression. (adapted from American Psychological Assn, 2000)
Behavioral health providers on the primary care team may be a nurse practitioner, a psychologist or a social worker. The goal of behavioral health care provider is to detect the behavioral health care needs of the patient being seen in primary care for the early identification and quick resolution of behavioral health issues to promote both physical and mental health. Patients may request referral to the behavioral health provider on the team at any time. Behavioral health providers can refer patients in primary care for weight management, stress management, brief interventions for anxiety and depression, tobacco and substance abuse programs, and alternative treatments for chronic pain.
