Transitional Residence Program — Housing for Homeless Veterans

The Community Mental Health Initiatives (CMHI) in addition to the Transitional Residence Program offer services including:
  • Homeless Outreach
  • Transitional Apartment Program
  • Community Residential Care Program
  • Homeless Food Pantry
  • Veterans Affairs Supported Housing Program (VASH)
Compensated Work Therapy (CWT) and Transitional Residence Partnership began in 1990, and have offered:
  • Referral to substance abuse treatment programs
  • Vocational rehabilitation services
  • A residential component
  • Psychosocial case management
The Residential Component
Transitional residences provide safe and sober housing. The program will help you develop independent living skills while transitioning to a productive life style in the community. You will learn to balance work with treatment. A house manager residences at transitional residence to assist you. The transitional residence coordinator will assist you in the coordination of your care to accomplish permanent housing in the community.

To be placed in one of these residences you must:
  • Be without a permanent home
  • Be willing to stay actively involved in your rehabilitation program
  • Be willing to participate in Veterans Industries programs
The Residences
Our homes offer you a safe and comfortable place to live. A house manager lives in each home and provides support and supervision. They are even available to help you evenings, weekends, and holidays.

While there, you will follow an individualized treatment plan that has been developed with your input. Your treatment plan will address your specific goals and objectives to be accomplished through your transitional residence program.

Shook House, Albany
Living accommodations for 3 veteran residents and 1 house manager.

Delmar House, Delmar
Living accommodations for 5 veteran residents and 1 house manager.

Lansingburgh House, Troy
Living accommodations for 3 veteran residents and 1 house manager.
