- Behavioral Health Practice: Offers individual and group counseling to veterans, as well as wellness workshops on a variety of topics including stress management, sleep hygiene.
- Home Based Primary Care: Certain patients with relatively short-term problems. These patients need healthcare services, home training, and home adaptation. Services are provided until the patient can be properly cared for in an outpatient clinic.
- Personal Care: Board certified physicians for primary, urgent, and specialty care, referrals to the Canandaigua VA Medical Center.
- Laboratory: Blood drawing and analysis services available.
- Prescriptions: As prescribed by VA providers, routine prescriptions processed through the mail. Emergency medicines provided.
- Radiology: Chest, abdominal, and extremity x-rays available on premises.

Driving Directions
The clinic is easily accessible by Route 390. From Route 390 North - Take Exit 16, East Henrietta Road (Route 15A). From Route 390 South - Take Exit 16B, continue straight through intersection at West Henrietta Road (Route 15) to East Henrietta Road (Route 15A).
Follow Route 15A North approximately 1/4 mile to Westfall Road. Turn right onto Westfall Road. Follow Westfall Road approximately 1/2 mile. Turn right onto Sawgrass Road. The VA Clinic is on the right.
Via the NY State Thruway, you may take Exit 46 to Route 390 North -or- Exit 45 follow Route 490 West to Route 590 South to Route 390 North.
Coming from the City of Rochester, you may take Routes 490 or 590 to Route 390, or take either South Clinton Avenue, or Mt. Hope Avenue to Westfall Road.
When traveling South on South Clinton Avenue, turn right onto Westfall Road. The clinic is approximately 1 mile on the left.
When traveling South on Mt. Hope Avenue (Route 15) or South Avenue, turn left onto Westfall Road. The clinic is approximately 1/2 mile on the right.
Bus Routes
Regional Transit Services (RTS) provides bus services on Route 11: Joseph/South Clinton at various times throughout the weekday. Call RTS at (585) 288-1700 for schedule information.
Buses originate at Main & St. Paul, Clinton & Goodman, Rochester Psychiatric Center, and Monroe Development Center.
