Adult Day Health Care (ADHC) is a therapeutically oriented outpatient day program, which provides health maintenance and rehabilitative services to frail elderly and functionally impaired veterans in a group setting. Each patient receives an individualized program of care that provides them and their caregivers the information and skills needed to manage the veterans care at home.

The Purpose of the ADHC Program is to:
  1. To enable frail elderly and functionally impaired veterans to reside in supportive home environments.
  2. To facilitate discharge planning and to reduce risk of readmission or institutional placement.
  3. To improve quality of life by maximizing the veterans physical, rehabilitative and psycho-social function.
  4. To provide support, education and respite for the family and other caregivers in order to enable them to maintain veterans in the community.
Adult Day Health Care Services:

  • Individualized Plan of Care Designed to Meet Elderly Veteran Health Care Needs
  • Coordination of Care with Primary Care Provider
  • Nursing Assessment & Care
  • Case Management/Care Coordination
  • Therapeutic Exercise
  • Leisure Activities
  • Health Education & Medication Management
  • Weight and Blood Pressure Monitoring

  • Therapy Provided by:
     — Kinesiotherapist or Physical Therapy Assistant
     — Recreation/Activity Therapist

  • Caregiver Support and Education
  • Intergenerational Programming/Visits/Activities
