Help us protect your privacy!
hand writing
In an effort to protect your privacy, you are required to sign a Release of Information Form every time your VA health care provider submits forms for insurance, compensation or medical reasons. The forms will then be sent to the Release of Information Office for processing.

Your VA health care provider is not permitted to release any information without your signed release form and the approval of the Release of Information Office. This procedure is being implemented to protect both patients and providers.

Moving? Getting married?
Don’t forget to tell us!

If you have changes to your mailing address, telephone number, marital or dependency status or employment status, please notify the Buffalo VA regional office as soon as possible. If you’ve already notified a VA medical center or community-based outpatient clinic, you must still notify the regional office. To update your information by phone, call 1-800-827- 1000, or you may mail your changes to Department of Veterans Affairs, Regional Office, 130 South Elmwood Avenue, Buffalo, NY 14202-2478.
Time to replace your Veteran Identification Card
If your Veteran Identification Card (VIC) still shows your Social Security number and date of birth, it’s time for an update. The newer VICs don’t include this sensitive information that puts your identity at risk. If you’re unsure whether you should replace your card, please ask at your next appointment.

More cemetery plots to be available soon

The Department of Veterans Affairs National Cemetery Administration offers free burials for veterans at any of the 122 national VA cemeteries with available space. All veterans are entitled to receive a flag, ceremony and government headstone or marker regardless of where they’re buried. To accommodate the growing number of deceased veterans, plans are under way to expand several cemeteries, including the Gerald B. H. Solomon Saratoga National Cemetery, which will gain an additional 25 acres. For more information about the Saratoga site, contact the cemetery at
(518) 581-9128.
